GROUP CLASS CANCELLATION & RESCHEDULING POLICY

BodyAligned has a strict 12-hour cancellation policy for all group classes. Please cancel

through your client portal or call 909.796.8400.

All cancellations < 12 hours will be charged in full.

PRIVATE PILATES, DUO PILATES AND PHYSICAL THERAPY PRIVATE

BodyAligned has a 24-hour cancellation policy for all Private and Duo Sessions.

All cancellations < 24 hours will be charged in full.

REFUND AND EXPIRATION POLICIES

All services are non-refundable. Packages must be used with the expiration date shown at time

of purchase of package.

RELEASE OF LIABILITY

In consideration of being allowed to participate in any way in the BodyAligned Physical Therapy & Pilates programs, related events and activities, I recognize the necessity of occasional physical contact with therapist/instructors and the risks of illness and injury inherent in any physical fitness program. I am participating upon the express agreement and understanding that I am hereby and releasing BodyAligned Physical Therapy & Pilates Center and its staff against all claims.

I HAVE READ THIS RELEASE OF LIABILITY AND FULLY UNDERSTAND ITS TERMS,

UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IN FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

Photo Consent & Release

I consent for photographs and/or video images to be taken of me by BodyAligned PT & Pilates. or a representative. I understand the images will be a part of my medical record and may be used for purposes of medical teaching or training or for marketing purposes (website, print, digital or social media). By consenting to photographs and/or video images I understand I will not be compensated from any party. Although photographs and/or video images will be used without identifying information such as name, I understand it is possible someone may recognize me. I further acknowledge that my participation is voluntary and agree that use of any photographs and/or video images confers no rights of ownership or royalties whatsoever. I authorize the use of photographs and/or video images: (please initial indicating YES or NO below)

For educational purposes (medical teaching or training)

For marketing and advertising purposes (website, print, digital, or social media)

At my request, my photographs and/or video images will only be used as part of my medical record.

Consent Signature

By checking, I hereby release BodyAligned PT & Pilates Center, its employees, and any third parties involved in the creation of or publication of educational or marketing materials, from liability for any claims by me or any third party in connection with my participation.

By completing this form, I confirm understanding of this consent. If I wish to withdraw my consent in the future, I may do so via written request submitted to BodyAligned PT & Pilates or by completion of a new form.
